Advance Care Planning

This community presentation discusses what is “Advance Care Planning” and why it’s important. How to think about the kind of medical care you might want in the future and how to discuss this with the people who need to know. What is a POLST and do I need one? What is an Advance Directive and do I need one? We’ll have Advance Directive copies on hand and will allow time to complete it, for those that wish to do so.
