The Oregon Physician Orders for Life-Sustaining Treatment (POLST) form has recently been updated. POLST is used to document medical decisions to be made in the event of a medical crisis in which you cannot speak for yourself. Gabriel Ledger MD is an emergency physician, Medical Director of Community Outreach Incorporated, and the producer/director of the documentary short film, “The Rest of My Life: Stories of Trauma Survivors.”
