What Services Do We Offer?
Hospice Care
A supportive approach to end-of-life care provided at the patient’s residence. Care is available for people of all ages, including infants and children, and for persons of any race or ethnicity.
Palliative Care
A medical program for individuals who are not under hospice care but are living with a serious or chronic illness . We offer interdisciplinary care for individuals through our Palliative Care Case Management program. We also offer clinical guidance to providers through our Palliative Care Consultation services.
Transitions
A non-medical community program to help individuals live well with a serious, life-limiting illness. We provide assistance in connecting with community services and navigating patients and family members through this difficult time.
Caregiver Support
For those caring for a family member or friend through a chronic or terminal illness. We offer ongoing caregiver education and support groups, and provide resources for family caregivers throughout the Mid-Willamette Valley.
Grief Support and Education
We offer a variety of bereavement services, including individual counseling, grief education and support groups, and remembrance events. Grief support is available to anyone in the Mid-Willamette Valley who is impacted by death.
End-of-life Education
We offer support to the community through training and presentations tailored to specific audiences. Topics range from advance care planning to dementia.
